Important Info:
1. A parent can not fill out the form because it is an SMUSD form and it is a student doing the email so it must be done through the student's email and it must be picked up during lunch. 

2. The Varsity letter is free if you have an ASB card.  If you do not have an ASB card the cost can be 1$ to 15$ depending on what you would like.
