SMHS SPORTS PHYSICALS: Students who wish to participate in sports activities during the 2025/26 school year will need to have a Sports Physical.  Sports Physicals may only be performed by a licensed MD, DO, PAC, and NP. Also, screening must be performed after June 1st, 2025 for the 2025/26 school year.

ONLINE CLEARANCE PACKETS: Students who wish to participate in a sport for the 2025/26 school year will need to have an Online Clearance Packet completed and turned in before you can practice and or try out.  This process is completely electronic.  Please electronically submit your sports physical as a attachment to your account in home campus.
